If I wish to cancel my policy, what should I do?
The policy owner should contact Trustmark's Worksite Claim Department. Generally the request can be taken over the phone. The policy owner must be able to verify the last four digits of their social security number, the correct address, their date of birth and name(s) of persons insured by the policy.
Some employers, agencies and third party administrators require written requests in order to stop the payroll deductions and cancel coverage. In these situations it is requested that the policy owner write a letter and include their policy and/or social security number, a statement of intent to cancel and the policy owner's signature. The request can be sent to Trustmark's Worksite Claim Department.
