Trustmark Life's Claim and Eligibility Look-Up for Insureds is a user-friendly system
with online access to payment status of current and previous
Trustmark Life medical and dental claims as well as eligibility/benefit information
for themselves and their family members.
After you register online to use the Claim and Eligibility Look-Up, simply enter your
Social Security number and Group ID number to access your secure claims records.
You can view the status for submitted claims and see each family member's eligibility, individual
and family deductible, and out-of-pocket amounts that have been satisfied.
For those times when you can't access the Internet, you can receive claims,
eligibility and benefits information over the telephone anytime by calling the
toll-free number on the back of your Medical Identification Card.
