You may click here to access Medical or Dental Claim Forms or contact your plan administrator at your employer. A medical claim form is not required in order to submit a claim. In most all instances, your healthcare provider will bill Trustmark Group Select directly for healthcare service and supplies you receive. If you need to file a claim, please mail a copy of the claim to the address found on the back of your ID card. Trustmark Group Select's claim processing services meet or exceed industry standards for claim turnaround, accuracy and response time.

Prescription Drug Benefits are included with all Group Select medical plans. Click on the Prescription Drug Benefits link to locate a network pharmacy, review the Performance Drug List of commonly prescribed medications or to refill a prescription.

Trustmark Life offers a LabCard program through LabOne, an innovative benefit that saves money for Plan participants by covering outpatient laboratory testing at a no out-of-pocket expense provided the tests have been ordered by a doctor, are covered and approved by your health benefit plan administrated by Trustmark Life and sent to LabOne for processing.

Trustmark Group Select has teamed with United Resource Networks (URN) to offer an Organ Transplant Program. Covered members receive a higher benefit at facilities that have greater experience and success with organ transplants.

MaternaLink®. MaternaLink is a maternity wellness program that encourages prenatal care and helps mothers-to-be take an active role in getting infants off to a healthy start. The program provides expectant moms with educational materials and resources about pregnancy, childbirth and baby care. The program is completely voluntary, confidential and available at no additional cost to members.
