Healthy Foundations is a suite of online services that integrates Trustmark’s consumer-directed health plans with online health information and decision support tools to help consumers make smart, informed healthcare choices.

Track the status of current claims, eligibility and benefits with our online Claims and Eligibility Look-Up system. It's a user-friendly system with online access to payment status of current and previous Trustmark Life claims as well as eligibility and benefit information for all covered members.

Are you looking for a doctor or a hospital near you? Check out our Physician/Hospital Look-Up feature, which allows you to directly access a list of hospitals and physicians from your network.

You may click here to access Medical or Dental Claim Forms or contact your plan administrator at your employer. A medical claim form is not required in order to submit a claim. In most all instances, your healthcare provider will bill Trustmark Life directly for healthcare service and supplies you receive. If you need to file a claim, please mail a copy of the claim to the address found on the back of your ID card. Trustmark Life's claim processing services meet or exceed industry standards for claim turnaround, accuracy and response time.

Trustmark Life offers Prescription Drug Coverage to insureds and covered family members. Coverage includes copayment and/or deductible options for outpatient prescriptions purchased at a network pharmacy or through a mail-order program.

The HSA Plan
If you are enrolled in the Trustmark Group Benefits HSA Plan, you can view your recent health savings account activity, access administrative forms or inquire about account investment options, all online. Visit the Healthy Foundations page for more information.

The HRA Plan
If you have coverage through the Trustmark Group Benefits HRA plan, click here to login to Healthy Foundations. You can check on claim reimbursements and review your HRA account balance.

LabCard
Trustmark Life offers a LabCard program through LabOne, an innovative benefit that saves money for Plan participants by covering outpatient laboratory testing at a no out-of-pocket expense provided the tests have been ordered by a doctor, are covered and approved by your health benefit plan administrated by Trustmark Life and sent to LabOne for processing.

Organ Transplants
