Trustmark Group Insurance partners with Caremark™ to offer prescription drug coverage. We offer two types of outpatient prescription drug programs that may be selected by employers: Tiered copay plans and Rx Price Assurance Tiered copay plans provide various deductible, copay and or coinsurance options. Rx Price Assurance is a prescription drug discount program through Caremark network pharmacies. Both tiered copay plans and Rx Price Assurance provide coverage for outpatient prescriptions purchased at a network pharmacy or through mail-order.

Tiered copay plans place drugs into tiers. Generic drugs are in the 1st tier (lowest copayment). Preferred brand drugs are in the 2nd tier (middle copayment) and nonpreferred brand drugs are in the 3rd tier (highest copayment). The copayment amount is determined by the prescription drug benefit plan selected by the employer. Some drugs may not be covered under your plan. Since many brand name drugs have generic alternatives available at a lower cost, our prescription drug three tier copay plans encourage generic drugs and mail order when possible.

Pharmacy Locator
For a list of Caremark participating pharmacies in your area, login to Caremark.com. A first-time visitor can register to gain access to their site. Once registered, use your login ID and password, click on the "Prescriptions & Coverage" tab, and select "Find Local Pharmacy" from the left side menu on your screen.

Prescription Refills and Mail Order
You may visit Caremark.com. to refill your prescription online. You may also print order forms. Simply log in to Caremark.com with your login ID and password, click on the "Prescriptions & Coverage" tab, then select "Print Forms & ID Card" from the left side menu on your screen. You can access other paper forms also, click here to access Mail Order and Other Claim Forms.

Performance Drug List
Prescription drugs on the Performance Drug List are available to you at a lower cost as part of your prescription drug benefit. The performance drug list is comprised of the most commonly prescribed drugs and serves as a guide for getting excellent values at a member pharmacy.

Drug Formulary
Drug formularies are comprehensive lists of prescription drugs selected by a committee of physicians and pharmacists. The committee determines the list using strict criteria such as safety and effectiveness. Trustmark Group Insurance uses Caremark's 500H and 500M drug formularies. Look at your medical identification card for the formulary number. Click on the appropriate link and select "Drug Tiers 500H" or "Drug Tiers 500M" from the left side menu for a complete list of drugs in the respective formularies.

Specialty Drugs
Trustmark Group Insurance has teamed with Caremark to offer a simple, reliable and cost-effective Specialty Drug program to offer injectable and specialty drug and supply needs. Caremark provides a fully integrated clinical management program designed to encourage appropriate drug utilization and competitive pricing. It's a convenient way for members to get their injectable drugs and supplies, while experts provide care services to help members with specific chronic conditions.
