Step 1. Participate in a Biometric Screening

Trustmark has partnered with HealthFitness to offer onsite biometric screenings at the larger offices during October. This is also the final weigh-in for Dave McDonough’s 10,000 Pound Healthy Weight Challenge. A voluntary biometric screening is a quick way to get an assessment of your basic health indicators. All employees — even those who are not covered by the medical plan — are invited to participate at no cost. The 30-minute screening is optional and private; Trustmark will not learn anything about your individual results.

You can sign up for your biometric screening by registering for the new Be Your Best Self health improvement program portal at www.liveforlife.net/hfit/trustmark. Click on “Login,” then click “New visitors click here!” You’ll be asked to enter your unique wellness program User Name, which is the first initial of your first name, up to the first four letters of your last name and the MMDDYY of your birth date. (Example: Anne Smith’s date of birth is 01/04/1946; her unique ID would be asmit010446). You will use this unique ID each time you access the portal. Once you’re logged in, click the “Health Scheduler” button on the home page to schedule a time for your onsite biometric screening. Employees in Mequon, Tinley Park, Overland Park and Wheeling will sign up manually in their offices.

Tests will include cholesterol (total, HDL, LDL, triglycerides), blood pressure, resting heart rate, blood glucose and body mass index. Fasting for 9-12 hours before your screening is recommended for the most accurate results, though you may drink water. If you are unable to fast, non-fasting ranges for glucose will be provided, although recommended ranges for non-fasting triglycerides and LDL are not available.

Immediately following the screening, you will learn what your numbers mean and also have an opportunity to speak with a health advisor. During this conversation, you will learn about additional program offerings made available to participants of the Trustmark Be Your Best Self health improvement program.

Employees who cannot attend an onsite screening may have their doctor complete the Alternative Means Screening (AMS) form, also located on the Be Your Best Self portal. Follow the instructions carefully and submit your form via fax or mail postmarked by Oct. 28, 2010 to complete this step for the incentive. After you submit your form, a health advisor will contact you about additional programs available to you.
