Step 4. Choose a “Wellness” Medical Plan Option on the ADP Website

Make your 2011 benefit selections on ADP Employee Self Service at https://portal.adp.com between Oct. 15 and 28. The deadline to enroll is 11:59 p.m. Eastern time on Thursday, Oct. 28.

Choose “Participate in Wellness” under the medical plan option to receive your discounted premium. Your username is the first initial of your first name and your entire last name and @TMK (for example, ASmith@TMK). You created your password during the registration process.

All employees are required to visit the site to select their benefits and, if applicable, their FSA and HSA contribution elections for 2011. Print a confirmation page for your records. If you experience login issues contact Pauline Porter at Pauline.porter@trustmarkins.com. No changes will be allowed after open enrollment ends on Thursday, Oct. 28.
